How does coal workers' pneumoconiosis present?

Most patients with simple coal workers' pneumoconiosis remain asymptomatic for many years. Mild chronic cough and sputum production may develop after prolonged dust exposure. Progressive exertional dyspnea occurs as pulmonary fibrosis advances.

Physical examination may initially be normal. Pulmonary function tests show restrictive impairment in advanced disease. Chest radiographs reveal multiple small nodular opacities, predominantly in the upper lung fields. Progressive massive fibrosis produces severe respiratory disability and pulmonary hypertension. Chronic hypoxemia may eventually lead to cor pulmonale.

The disease progresses slowly even after exposure ceases in some patients. Prevention through dust control remains the most effective strategy.
